Type of Request:

Proposed New Construction
Any proposed structure which will be newly constructed from the ground up, such as monopoles, lattice towers (self-supporting and guyed), buildings, water tanks, smokestacks, etc. Does not include a proposed antenna addition, height addition, lighting rod addition, etc., to an existing structure.

Existing
A completely built structure. May include appurtenances existing on the structure.

Co-Location
A completely built structure supporting an FCC licensed transmission antenna/emitter and upon which a differently licensed transmission antenna/emitter is proposed to be attached or is attached.

Alteration
A modification to a structure. May be an existing structure or a proposed new structure. Includes corrections to heights and location, addition or reduction of heights due to antennas, lights, lighting rods, addition or removal of transmitters, change of frequencies or ERP, modification of marking and lighting, etc.

Follow-up
Additional work request related to a previous Airspace Design work order.
Company Purchase Order Number
Airspace Design prefers that each work request be accompanied with a purchase order number obtained by the requestor from his or her company.
Prior Airspace Design Study Number
If the work request is related to previous work conducted by Airspace Design (a follow-up), state the original Airspace Design study number assigned to the previous work by Airspace Design.
Prior FAA Study Number
If the work request is related to a structure which has been studied by the FAA, state the FAA Aeronautical Study Number assigned to the structure by the FAA.

If first-time requestor, please contact Airspace Design at 678-924-8000 with invoicing and shipping address.

Airspace Design prefers that work requests be made through the Internet utilizing the "Work Order Data Sheet" on Airspace Design's Web Site.
Site Number/Name
Include both the number and name, if available.
City/State
The nearest City and State in which the site is located.
Latitude and Longitude
Rounded to the nearest .01 seconds.
NAD 83 or NAD 27
Refers to the specific North American Datum Reference of the latitude and longitude. Each survey or GPS derived set of coordinates needs to be referenced to NAD 83 if possible.
Site Ground Elevation
This is the ground elevation at the site of the coordinates, rounded up to the nearest WHOLE foot. Do not use for search area studies.
Desired Total Structure Height Level
This is the above ground height (AGL) of the overall structure, rounded up to the nearest WHOLE foot. Does not include the height of the site ground elevation, but does include any additional height due to appurtenances, etc. This may or may not be your antenna tip height.
Airspace Design Opinion Letter
A letter containing an expert opinion concerning FAA notice or FCC mandated FAA notice requirements, marking and lighting requirements, and a statement that the FAA would not or does not have an objection to the structure. This letter may be extremely useful at the local zoning level and/or for site documentation.
State Notice
Some states require notice of proposed construction or alteration of structures. Airspace Design provides this service as an additional product to the site specific study. Do not request this service with an original search area study.
AM Broadcast Station Search
FCC rules protect existing AM Broadcast Stations from possible adverse effects caused by emissions from newly constructed cellular radio towers. Airspace Design identifies AM stations which would impact the proposal as an additional product to a site specific or search area study.
FAA Form 7460-1
Notice of Proposed Construction or Alteration form used to file a structure, etc. with the FAA. Includes an Airspace Design addendum and site plot on an excerpt of a 7.5' topographical chart (scale 1:24,000). Airspace Design provides this service as an additional product to the site specific study. Do not request this service with an original search area study.
Frequency(ies)
A frequency band should suffice (800-900, 1930-1945, etc.).
ERP = Effective Radiated Power, indicate Watts or dBm.
Name, Title and Phone Number of Person Signing
Airspace Design would prefer to have a scanned signature on file to use with our 7460-1 software. Please contact Airspace Design at 678-924-8000 for additional information on this service.
Airspace Design Study Number
If the work request is related to previous work conducted by Airspace Design (a follow-up), state the original Airspace Design study number assigned to the previous work by Airspace Design.
FAA Study Number
If the work request is related to a structure which has been studied by the FAA, state the FAA Aeronautical Study Number assigned to the structure by the FAA.

This is the latitude and longitude of record for the structure, rounded to the nearest .01 second. If no previous recorded data is available, leave blank.
NAD 83 or NAD 27

Site Ground Elevation
This is the ground elevation of record of the structure at the coordinates of record. If no previous recorded data is available, please leave this section blank.
Total Structure Height
This is the highest point of record on the structure. This is the above ground level height (AGL) of the highest point of record of the overall structure. This does not include the height of the site ground elevation. If no previous recorded data is available, please leave this section blank.
